Waiver and Release of Liability. I, the undersigned parent or legal guardian of the child(ren) named above, hereby voluntarily agree to the following: 1. Supervision Acknowledgment: I understand that babysitting services are being offered as a courtesy during the community event, and will be staffed by volunteers and/or temporary caregivers who may not be professionally licensed childcare providers. 2. Health and Emergency Care: I certify that my child(ren) are in good health and have no medical conditions that would prevent safe participation. I authorize the caregivers to administer basic first aid and to seek emergency medical treatment if necessary. I agree to be responsible for any costs associated with such treatment. 3. Release of Liability: I hereby release and hold harmless SUKOON SOFLO, INC. and JAMAAT UL MUTTAQEEN, INC, its officers, volunteers, agents, and representatives from any and all liability, claims, or demands arising out of or related to any loss, injury, or damage that may occur while my child(ren) are under the supervision of event babysitting staff. 4. Pick-Up Policy: I agree to promptly pick up my child(ren) at the end of the event or if requested by the staff due to behavioral or emergency concerns. 5. Behavior Expectations: I understand that if my child(ren) become disruptive or pose a risk to themselves or others, they may be removed from the care area, and I may be required to retrieve them immediately. 6. Personal Belongings: I understand that any personal items brought by my child(ren) are their responsibility, and the event staff are not liable for lost, damaged, or stolen items. 7. Assumption of Risk: I, the undersigned parent or legal guardian, understand that injuries or accidents may occur while my child(ren) is/are under the supervision of the babysitter. I voluntarily assume all risks associated with such care.
